Our fracture liaison service is a care-coordinated, multispecialty service championed by Orthopedic Surgery. Our liaisons utilize the support of local health care providers and our orthopedics team to coordinate with your primary care physician to improve your quality of bone health and clinical outcomes.
We work with your primary care physician and specialists to provide the following care:
- Identify those at risk for falls
- Osteoporosis
- Osteopenia
- Blood work to evaluate bone health
- Medication management
- Specialized fall risk exercises and programs
- Referrals to Physical Therapy and Specialists
- Diet, exercise, and supplement recommendations
- Bone Density Scan orders
