Position Title: Case Manager

Position Purpose:Provides for the development, coordination, and implementation of care management services over the continuum of care. The position will incorporate the traditional functions of Discharge Planning, Quality, and Utilization Management with a focus on high-risk outpatient populations.
Minimum Qualifiactions: Registered Nurse currently licensed to practice in the state of New Mexico.
Bachelor’s degree in Nursing or 3 + years experience in nursing required. Experience in Care Management preferred.
Minimum of 20 hours professional continuing education per year for the prior three years.
Five years recent relevant clinical experience in an acute care facility required.
Current knowledge in various levels of patient care and utilization of clinical protocols and care paths.
Strong interpersonal skills and ability to work with patients, families, peers and other departments required.
Able to use word processing and computer spreadsheets.
Excellent communication and problem solving skills.
Strong patient education abilities.
